Tryouts for fall high school sports are just around the corner, and there are a few things student athletes need to do before they hit the field (or course or court).

Prior to participating in any sport, Park City High School (PCHS) requires that students turn in the following items to the main office: athletic fees or fee waivers, physical evaluations, contracts and tryout checklists. These forms are available at http://pchs.pcschools.us/index.php?page=260 or at the front office of the high school.

Fees

The fees for the 2009-2010 school year are $85 for football and $60 for all other sports. Fee payments help cover the costs of equipment, uniforms, referees and tournament entry fees. Each team also host fundraisers to supplement its budget, explains PCHS Athletic Director Doug Payne.

Fee-waiver applications are available for those who qualify, which includes anyone on the free or reduced-lunch program and anyone who falls within a specified income level. For more information on fee waivers, contact Barb Swenson at 645-5650.

All checks should be made payable to Park City High School and delivered to the front office. Fees are collected after a student is selected to a team and must be turned in before the first game or meet.

Physicals

Before they try out, athletes must complete and present proof of a physical evaluation. The physical examination form (Form A) must be completed by a doctor, physician’s assistant or registered nurse practitioner, and the clearance form (Form B) must be completed by a parent or guardian.

Parents and students must sign a disclosure and consent document verifying that they recognize the inherent risk of participating in athletics and that they consent to treatment deemed necessary by health-care providers for any illness or injury resulting from participation.

Physicals are valid for one year from the date of the physical. They must remain valid for the duration of the sport that the student is participating in.

Contracts

Athletic contracts must be completed in full and signed by parents and students prior to participation on a sports team. The contract basically states that the athlete will abide by athletic policies and rules as defined by the Park City School District and the Utah High School Athletic Association (UHSAA). These include academic requirements, attendance regulations, and rules regarding drug, alcohol and tobacco use. Parents must sign a code of conduct to act responsibly at sporting events and practice good sportsmanship.

The contract also covers insurance information. The Park City School District does not carry accident/injury insurance. Athletes are not required to have medical coverage, but those who do not have insurance are responsible for any medical costs associated with sports-related accidents or injuries.

Insurance for student athletes is available for purchase prior to Oct. 1 through a local provider. "There’s a special policy for athletics, and it’s a really good price," says Payne. For more information, contact the district office at 645-5600.

Tryout checklist

The UHSAA mandates that tryout forms be completed in full and signed by parents, athletes and coaches. Coaches will provide these forms to athletes before tryouts commence.

The checklist ensures that athletes understand and agree to abide by the UHSAA eligibility rules and standards. Athletes must fill out a new form for each sport they intend to participate in.
